Tiffany Chohfi MSN, RN is a Project Director at the Colorado Center for Nursing Excellence. She received her Bachelor of Science in Nursing degree from Grand Canyon University in Phoenix, Arizona and her Master of Science in Nursing leadership and education degree from Walden University in Minnesota. Tiffany has extensive clinical and acute care experience. She leads the NEPQR-BHI grant, funded through HRSA, focusing on implementing behavioral health specialists in primary care clinics in rural Colorado. Through creative innovation, evidence based models and practice, she leads primary care clinics to behavioral health integration through team and practice transformation, training, coaching and supervision of clinic goals and sustainability. This has led to an abundance of mental health access in rural communities of Colorado leading to better patient outcomes and equitable care for people in rural and underserved areas.
